NobleCare offers two plans — NobleCare COMPLETE and NobleCare LITE — monthly subscriptions to helping you understand and navigate your care between doctor visits that goes beyond coordination.
NobleCare specializes in guiding older adults and their family through the twists and turns of care between doctor visits. I do not replace their doctors—I help connect the dots between them. When specialists give multiple instructions or medical terms feel like another language, I am here to translate, simplify, and help older adults and their family understand their options.
I am available for those quick five minute or 45 minute consults when you need a reminder, reassurance, or have questions. If I do not have the answer, I collaborate with your doctors and facilitate getting you an answer before your next healthcare appointment. You deserve support beyond the exam room!
"My 76-year-old sister has been living with dementia for the past few years. Last year, she was moved into a memory care unit where she was still talking on the phone, engaging with family, and participating in activities.
But over the last few months, everything changed—she stopped eating, barely communicates, and no longer seems interested in seeing even her grandchildren.
It felt like she was put on hospice because everyone had given up on her.
What I couldn’t understand—what was keeping me up at night—was how dementia alone could lead to hospice at 76 years old.
Rachel helped me understand that my sister’s symptoms and functional decline were an expected part of advanced dementia. She helped me make sense of it all, and it felt good to be understood.
Talking to Rachel gave me the clarity and reassurance to feel more confident that my family was doing the right thing—not giving up, but making sure my sister was receiving the care, dignity, and support she needs right now."
